A case of Cerebral palsy with Cortical blindness


The child is a 7 years old male child suffering from Cerebral palsy with cortical blindness.The parents had come to us so that we can help in doing something for the childs irritability.The Child is always irritated and restless he is not comfortable in any position.The only thing that will make him comfortable is Music which is familiar to him.On listening to it he will keep quiet.

The child doesnt have any co ordination and he has no control over his urine and stools.The child cannot walk or crawl there is absolutely no co ordination present to add to this the child has no vision.The parents had observed that whenever his favourite television programme came he used to move his neck and his legs.

He can make out between bright and dark objects but cannot comprehend.When he is in a happy mood he will be on his own and will make noises ,will shout and laugh.He will also sing in his own way and during such times he will like to be appreciated.When other children come to his home he likes it.

Sometimes he will wake up in the morning in a happy playful mood will sing and he will be enjoying when he is alone.He is very sensitive to noise especially any sudden noise like the pressure cooker whistle he will get scared and has to be explained about it.

If he is angry he will express his displeasure about the same if he is not attended then he will bite the person.He is very attached to his father and will listen only to him.

The other aspect was he wanted to be in a big place and be comfortable he feels better when he is sleeping in supine position or on his sides.There is lot of drooling of saliva when he is sleeping.

During the pregnancy the mother had hypertension from the second month.Due to this she was advised to take complete bedrest as during the first pregnancy she had lot of problems.Also during this period she was very unhappy as she had not accepted the pregnancy. She wanted to work as a teacher and do some saving but this not happen. Also there was no support from the in laws which made her very sad and lonely.

Personal history

Cravings: Pickles, Ice-cream, crispy salty things.

Perspiration: on the scalp and the neck till the age of 3 years

Thermals
Sun: face becomes red
Coverings: he kicks off covers
Thirst: doesn’t ask for water
Urine: there will be one stop after the first part is over

Hair loss on the scalp in 2 places.

Child doesn’t not fixate or follow light in both eyes. Pupils were brisk in both the eyes. Torch light examination of the anterior segment was normal in both eyes. Digital tensions and fundus examination is normal in both the eyes. The child has agnosia due to cortical blindness .Poor visual prognosis

Ct.scan brain: hypoxic encephalopathy
Sub-archenoid hemorrhage

On examination
Laxity of the body
Thinning of the body
Multiple enlarged sub mandibular glands.
Head is small and irregular
Gums are swollen
Drooling of saliva.
No co ordination

Treatment

1) Bufo 30 dissolved in water 3 doses

Stools are proper in the morning.
Sleep is better
